(These forms are used for original and renewal applications):
- Alcohol Beverage License Application (AB-200 Fill-In Form)
- Alcohol Beverage Individual Questionnaire (AB-100 Fill-In Form):
This must be completed by each individual applicant, by each member of a partnership, by each officer, director, and agent of a corporation or nonprofit organization, and by each member/manager and agent of a limited liability company. - Alcohol Beverage Appointment of Agent (AB-101 Fill-In Form) For LLCs or Corporations, or Nonprofit organizations only
- For New Applicants: Sole Proprietors, Agents, or all partners of a partnership must also provide. Proof of completion of the “Responsible Beverage Server” course accredited by the State and taken within the last 2 years
- Instead of the course, the applicant may provide proof that they have held an operator’s license in another municipality in the State of Wisconsin within the last two years or have been an agent at a licensed establishment within the last two years. - Copy of Current Wisconsin Seller’s Permit
- $10 Publication Fee. (Final Payment will be determined on issuance date).
Other considerations:
- Renewal applications are due April 15.
- There is at least a 15-day waiting period from the date the application is submitted to the issuance.
- All licenses are granted by the Town Board, which meets the second Tuesday of the month.
